Eligible persons

Heirs or any Beninese or foreign citizen who witnessed the death and holds a civil-status document or, failing that, the deceased's identifying information

Death not attested by a healthcare worker (typically deaths at home)

  • Death confirmation attestation + Death declaration form (collected at the borough of the place of death)
  • Any document bearing the deceased's NPI (if they had completed RAVIP)
  • Any document bearing the applicant's NPI
  • Secure marriage certificate if the deceased was married.

Death attested by a healthcare worker

  • Death certificate + Death declaration form (issued by the hospital)
  • Any document bearing the deceased's NPI if they had completed RAVIP
  • Any document bearing the applicant's NPI
  • Secure marriage certificate if the deceased was married.
