Birth certificate with marginal note

Birth certificate to which legal information arising after the person's birth (marriage, divorce, simple or full adoption, or any other judicial decision affecting civil status) is added in the margin of the document.

• Proof of the events justifying the requested marginal note (marriage certificate, divorce certificate)
• Or court decision (e.g. in case of name and/or surname change)
• Any document bearing the NPI of the certificate holder.
